Our care management team can help you navigate referrals and specialist visits, finding transportation to and from your appointments, and navigating the complicated health care system.
Share Ourselves is recognized by some insurance companies for our Enhanced Case Management. Enhanced Case Management for some designated services helps to ensure we are supporting some of the most challenging aspects of your health journey.
